Skilled Nursing Evaluations
An evaluation from the nurse is important in determining the level of care that you need at home. We first conduct a consultation with the patient and the family and then propose a care plan (as approved by the physician) that will outline the health management needs of the patient.
Observation & Assessment
We will observe your day to day health conditions and assess your progress with the care plan. We also report to your physician for significant changes which can be determinants to adjust your care plan to promote optimum health care results.
Catheter Care
We have well-trained nurses who can conduct basic and complex nursing care including Catheter Care. To prevent infection and to promote faster recovery, your nurse will help you maintain your catheter.
Cardiac Rehabilitation
For heart patients who may need anti-coagulation therapy, medication management and vital signs monitoring, we have well-trained nurses who can assist in your complex health management.
Congestive Heart Failure (CHF)
Patients with CHF need regular monitoring and immediate access to a nurses’ assistance in case of emergencies. We offer convenient home-based care for patients with congestive heart failure so that the patient gets a chance to still live as normal a life as possible at home.
